Strategic Plan for Forensic Customers

Illinois Department of Human Services, Division of Mental Health
IHDD worked with IDHS’s Division of Mental Health to develop a strategic plan to improve the State’s ability to provide more efficient and effective recovery-oriented care to its forensic customers. The strategy established four interconnected strategic goals and outlined several clear initiatives, which came together to create a multi-year roadmap and implementation plan.
Hypertension Action Plan

UI Health Department of Internal Medicine
With support from the BCBS Health Equity Pilot Program grant, IHDD has worked to design a suite of programs for socially-vulnerable hypertensive patients and their providers. These programs – which include one-on-one coaching and a hands-on cooking class – are being piloted and implemented across UI Health.
Participant Experience for Long COVID Study

National Institutes of Health
RECOVER is a national study to understand, treat, and prevent Long COVID. Alongside the study team, IHDD has tailored the study experience to the needs of people and communities affected by Long COVID. Clear study materials, quarterly update events, and transportation to in- person visits aim to inform, engage, and support participants from beginning to end.
Healthcare Transformation Collaboratives

Illinois Department of Healthcare and Family Services
Since 2020, IHDD has supported the $1.05 billion Healthcare Transformation Collaboratives (HTC) program with preliminary qualitative and quantitative research, strategy and storytelling, and human-centered implementation assistance. The HTC program aims to seed collaborative, equity- and community-driven innovation in Medicaid-dense areas of Illinois.
Patient Messaging Workflows

UI Health Department of Ophthalmology
IHDD helped the Department of Ophthalmology establish a team-based approach to managing hundreds of patient messages the department received per day. The new system distributed the workload across roles and standardized efficient workflows for each message type; as a result, message response time decreased significantly, as did the number of messages unresolved for more than two days.
Participant Retention Support for Early COVID-19 Study

National Heart, Blood, and Lung Institute
ACTIV-4b was an early- pandemic clinical trial to understand whether people recovering at home from COVID-19 were at risk of blood clots. Potential study participants were facing a frightening diagnosis, so IHDD designed streamlined processes and materials to promote confidence in the study and visibility into what they could expect next.
